Question

A home girl named Dianna from El Segundo Barrio is admitted into the Emergency room. She and her friends had been to a rave. Her friends noticed that she started twitching and having muscle spasms, which progressed until she was stiff as the board she was carried on. The nurse (you) upon examination observed an increase in muscle tone and hypereflexia involving all the limb and facial muscles. In her pocket were several pills yellow with dark flecks. Upon laboratory analysis it was discovered that it was a mixture of ecstasy and strychnine. Ecstasy although can cause muscle wasting and hallucinations it was not the cause of muscle spasms. Strychnine on the other hand blocks glycine receptors and could cause all the symptoms. Explain How? Clue: ACh is excitatory.

Explanation / Answer

This may be possible she had taken Ecstasy first followed by Strychinine or she had been taking Ecstasy for a longer time. Ecstasy is well known hellucinogic drugs used by adults for distress themselves. It is a neurotoxic drug having a same effect as cocaine and marijuana. However, Strychinine is a kind of pesticide used for killing rodents and small birds in the field. The symptoms above defined in question completely belong to Strychinine toxicity.
