1 - Park is equally responsible for the mishap that has happened due to Sandra's behaviour. Although the riders were informed via Public address system and signs, the ride was not designed safely, it had provision to stand. If this was eliminated the mishap could have been avoided. Hence park is equally responsible for the mishap.
2 - The park management could take few following actions to avoid such incidents
3 - if proper instructions are not provided to the guest or the owner do not care about them, such mishaps are bound to happen. For instance, if we own a restaurant, where food is served hot with help of knife, which both if not handled with care are dangerous. The guest could burn or cut himself, to make sure that doesn't happen owners must keep blunt knives, hot food to be served by servers only who are properly trained.
