Across the US, trauma/ER units are identified as a designation process and a verification process. Trauma Center designation is a process defined and developed at a state or local level. The criteria for categorizing trauma centers are carried out by state or local municipality. These categories are not the same across the states and are typically defined by a legislative or a regulatory authority.

Trauma Center Verification is an assessment activity undertaken by the American College of Surgeons (ACS) to improve trauma care. As per ATS(2017), the ACS verifies the presence of the resources listed in Resources for Optimal Care of the Injured Patient.The agency responsible for these functions is the American College of Surgeons (ACS) Committee on Trauma (COT).

As per the ATS(2017), the levels are

Level 1 - Level I Trauma Center is an exclusive regional resource that is a tertiary care facility central to the trauma system which is capable of providing total care for every aspect of injury.
