Scenario: The monthly report on medication errors was presented to all units within ECHO Hospital. Although the rate has decreased, staff identified that the primary cause of medication errors continues to be interruptions while preparing and administering medications.
The nurse manager has asked the staff members to offer assistance in solving the problem of preventing interruptions while RNs are preparing and giving medications.
Using the assigned readings, information from the literature and the above scenario, your knowledge of the, principles of safe medication administration create a discussion post in response to the following:
Suggest at least three (3) changes that can be made to prevent medication errors due to interruptions.
Select one (1) suggested change and write an outcome statement (realistic, measureable and contains a time frame).
Select one (1) suggested change and outline the steps that would be taken to implement it.
Describe how the selected change will be evaluated to determine its effectiveness in achieving your stated measurable outcome.
Explanation / Answer
Three changes that can be made to prvent medication errors due to interruptions are :
(a) Staff that prepares and administers medications should be having a separate room so that people do nnot enter the room and their specific medicine would be provided to the patients.
(b) There should be cooperation from other staff to help the htem administer medicines and entry of patients should not be allowed but the medicines would be provided to them directly.
(c) Nurses should be provided with the responsibility of delivering specific medicines to the required patients who wait outside but do not enter that room.
If we keep a separate room in which entry of patients is not allowed then there would be no chaos and the patients would get their medicine on first come first serve basis which in turn removes case of any complaints or interruptions.
The change is already discussed above and the steps required are also provided. This process would prove effective as there would be less chance of wrong medicine administration due to lesser or no interruptions.
