Question
5 Sadaf has an ulcer of her large intestine that has perforated, meaning that it has developed a hole that has gone through all tissue layers. a Through which tissue layers would the ulcer have had to pass to perforate completely? A major concern with large intestine perforation is peritonitis, or infection of the peritoneal fluid. Why is this a particular concern with the large intestine? b Why could peritonitis have wide-ranging effects on Sadaf's other organs of the abdomen and digestive system? Are there any organs that would be unlikely to be directly affected by peritonitis? Explain. cExplanation / Answer
A) There are four layers of large intestine:
1. Serosa, the outermost layer,
2. Muscularis
3. Submucosa
4. Mucosa, the deepest layer of large intestine.
B) Peritonitis is a major concern to large intestine, as it could cause abdominal sepsis which could cause circulatory shock and ascites.
C) Peritonitis could cause inflammation of abdomen (ascites), abdominal tenderness, dehydration due to loss of body fluids, stomach pain which can range from savere to even worse. A rupture in the large intestine could be a major source of infection.
Spontaneous bacterial peritonitis occur due to accumulates of fluids in abdomen which gets infected. Three fourths of the time, spontaneous bacterial peritonitis is caused due to aerobic gram negative organism such as E.coli.
