Question
You discover a mutation in anti-IAPs in a tumor sample. The mutation is within the IAP binding domain. You are trying to figure out whether you should treat this patient with a drug that is designed to induce apoptosis. What should you do and why?
A.Dont treat the patient with the drug, we dont want to induce apoptosis in this patient, they are already sick enough.
B.Treat the patient with the drug, it will work to induce apoptosis in the tumor cells and this is a good thing
C.Treat the patient with the apoptosis inducing drug because they should be very sensitive due the the mutation discovered in the ani-IAP gene. You expect apoptosis will occur at a high frequency.
D.Dont bother. The patient will be resistant to the drug since the mutation will prevent activation of the intrinsic pathway.

Explanation / Answer
Apoptosis resistance is a hallmark of many solid tumors, including several cancers, and is therefore, an important target mechanism for controlling cancer proliferation. The inhibitors of apoptosis (IAP) is a familu of proteins containing one or more conserved cysteine and histidine-rich residues. Recent researches have established diverse roles for IAP and showed they regulate apoptosis through the modulation of NF-kB signaling downstream of several TNF family receptors and to play an essential role in modulation of FAS-induced cell death. Therefore, the patient can be treated with apoptosis inducing drugs.
The answer is (c).
