A 60-year-old African American woman with metastatic ovarian cancer is experiencing chronic pain. Her pain ratings have been between 3 and 7 on the 0-10 numeric rating scale. Her vital signs remain within normal ranges. She is fully able to participate in her daily care. She describes herself as the matriarch of her family, and looks forward to daily visits from her family even when her pain rating is high. Her church pastor calls and visits her several times per week. She anticipates discharge in a few days. Initial Discussion Post: For your assigned scenario, address the following: 1. As.you see the patient history, most important heardles in her diagnosis is the old age and second one is she is emotionally connected with the church culture. Whenever patient in old age it is very difficult to treat him because of patient non compliance and the problem of dosing and adverse drug reaction.
In this patient i observed following 3 patient problem or intervention
1. Old age may create non compliance
2. History tell ahe is unable to take proper rest
3. Management of pain
Rationale behind my 1st intervention is that cancer is in metastatic stage that is if you treat one it develop in another way and it is fast growing and in this scenario we have to provide firm treatment to her either on anticaner drug or biotech product but they are very lethal and as this lady is having 60 year of age it is very difficult to manage the plan.
2. She is bind with culture perspective which is major heardles in her treatment , daily she has to go church for prayers and she anticipate also means there is some gap for treatment. She suffered from pain also and in this condition it is must for patient to take rest because cancer is critical disease.
3. Management of pain is somewhat difficult. If you put her on local analgesic , it is of no use because it do not low her pain, might be she require high dose of potent analgesic and after getting that one she should be in state of lethargy for that rest is very necessary but history tell us she is unable to take full fledged rest.
Priority for nursing pain is that we have to understand her about the pain state and our plan. We motivate her that if she rest then it is good for her health.
