What is pressure ulcers?

It is the injuries to skin and its underlying tissues due to prolonged pressure on the skin , which is also known Bedsores . It is mainly found near the bony areas of body such as heels, ankles, hip and tailbone.

Prevention of pressure ulcer :-

The prevention of pressure ulcer is by identifying at early stage, and by treating the ulcer and provding health education to the care provider and the pateints.

Steps for practice based evidence process in research are as follows.

Each and every nurse should use the Braden Scale for the identification of the pressure ulcers , atits early stage so that it is prevented.

B. 2 chairfast

C.3 Walks occasionally

D. 4 walks frequently.

Mobility score:- A. 1 completely immobile

B. 2 very limited

Nutritional score:- A. 1 very poor

B. 2 probably inadequate.

C. 3 adequate

D. 4 excellent

Friction and shear:- A. 1 problem

B. 2 potential problem

C. 3 no apparent problem.
