When people are faed with terminal illness and death, they usually need support and guidance to identify how they want to live the end of their life. The RN can plaly a vital role in this advance care planning process. Ellen Goodman started what is called "The Conversation Project" in 2012 to champion this need.

Identify and describe how as an RN you can promote and support this end of life decision process.

RN should be well aware about the patients condition. Using Hildegard's Paplau theory a RN gains interpersonal replationship with the patient. No matter what is the patient's wishes are an ideal RN will respect that decision and can only advise what is good for the patient. A RN can never force a patient into doing something they can only guide them. RN will observe patient's behaviour, diagnose according to the objective assessment, plan a care accordingly and implement it. For example if a patient complaints of being left alone and feels that she is not contributing into anything a RN will plan a group therapy for her and also a recreational therapy respectively. It is very important to listen to patient, Listening plays important role. After all the assessment, dianosis, planning and implementation a RN will evaluate her work and record it.
