When people are faed with terminal illness and death, they usually need support and guidance to identify how they want to live the end of their life. The RN can plaly a vital role in this advance care planning process. Ellen Goodman started what is called "The Conversation Project" in 2012 to champion this need.

Nurses are obliged to provide comprehensive and compassionate end-of life care.This includes recognizing when death is near and conveying that information to the families.The nurse should listen to the patient's talk about their values and concerns.This is the time the nurse play a unique role in taking care of both patients and their family members.

Nurses are in the position in advance care planning of patient and their wishes and whom the decision maker is.Nurses can make a major contribution in pallative care setting.Nurses tasks include assesing the patient for pain and other distressing symptoms and to alleviate them eventhough it may not improve the quality of life.Nurses also meet the psychological and spiritual dimension of terminally ill patient.

Nurses must have knowledge and communication skills to explain to patients and other family members,why CPR, medication and other therpy cannot help this patient and therefore should not be provided.Always document any conversation with patient or families and other health care professionals about end of life decisions to protect yourself against potential liability.
