Your patient is a 33 year old who went to the emergency department because of severe right flank pain. The patient is doubled over on the bed, writhing in pain. In addition, the patient is nauseated and has vomited twice. Your patient works outside as a flag person during the road construction season. The patient drinks at least 4 glasses of milk a day, eats lots of beets, nuts, and spinach.
a. what would you expect the diagnosis to be? Dicuss the pathophysiology that has occured?
b. What is the relationship between pain and vomiting with the patient's scenario?

As the patients has severe right flank pain which is mainly at the upper part of abdomen and the pelvis. As the patient is suffering from severe pain in the abdomen. there is a possibility of either Kidney stones. This is because the patients has nauea and vomiting. The main reason for kidney stone development is due to the high intake of calcium and vitamins in diet. Milk, nuts and Spinach has high content of calcium and different vitamins. As he works in as a flag person during the road construction season and may suffer from dehydration as he is exposed to sun all day long. Due less intake of water or due to loss of excessive water as a sweat enhance the process of formation of stones in kidney. The kidney stones are mainly formed due to the crystalisation of calcium and other vitamins and salts. Less water or body fluid in the body enhances the stones formaton in kidney.
The expected diagnosis can be done by various biochemical tests of the patients urine and blood. Kidney stones can be more confirmed by radiological study as C.T. scan or U.S.G.
The occurance of pain due to the the effect on kidney function. In this case the vomiting my occur due to the intake milk which may not be digested properly and causes pain in the abdomen.
