An 8-year old child was admitted to the hospital with body aches, fatigue, cogh and fever. He was diagnosed with a viral infection that spreads via large droplets. The nursing diagnosis is Ineffective Thermoregulation.

        1. Describe the precautions and personal protective equipment required to care for this patient.

        2. Identify five (5) nursing interventions for this patient who has a nursing diagnosis of Ineffective Thermoregulation.

                 2a. Discuss infection control practices for each of the five (5) nursing interventions that you have identified for this patient.

Explanation / Answer

1. To care for this patient the required precautions and personal protective equipment are given as follows:
1)   Using face mask as this will deny portal of entry to microorganism and stop the infection from spreading.
2)   Perform and comply with aseptic work practices in the surroundings of the patient.
3)   Monitor and follow decontamination practices so that transmission of microorganisms from patient to others can be prevented.

2. 5 nursing interventions for this patient will be:
1)   Place the patient in a room with warm temperature environment.
2)   Monitor signs of hypothermia which include weakness, discolored skin and fatigue.
3)   Avoid situations which can cause patient to lose heat. (Such situations include patient’s exposure to cold air or shower).
4)   Eliminate or reduce the sources causing heat loss in the patient. (Such sources include wet clothing, AC).
5)   Evaporation from body should be prevented.

2a. Practices for these interventions will be as follows:
1)   For bathing, a warm environment should be provided or bathing should take place under source of heat. The time patient is in contact with wet clothing should be limited.
2)   Air conditioners, fans and windows must be avoided near patient.
3)   The articles used for care must be warmed. These may include stethoscope, bed sheets, clothes, etc.).
4)   Heat absorbing objects specially metal objects must be limited in the room of patient.
5)   Washing and drying should be done in sections in order to reduce evaporation.
