ALT is an intracellular cytoplasmic enzyme. It is widely present in body tissue, with maximum in liver and kidney. It is a key enzyme in gluconeogenesis, which catalyzes the transfer of amino groups from alanine to 2-oxoglutarate.

The ALT reaction involves an amino acid and an oxoacid both as substrate and products. Quantitation makes use of the formation of the oxoacid (pyruvate), using lactate dehydrogenase to reduce it with NADH to form lactate. Thereaction is followed by measuring the decrease in absorbance at 340n.m.
