Medical Record Completion Policy

Purpose: To ensure timely completion of documentation for a patient in order to protect integrity of the electronic health record.

Policy Description

•Within 72 hours of patient visit, Medical records must be locked.

•If not locked within defined timeframes, Disciplinary action will be taken as per the definition of Medical record Delinquency policy and procedure.

Procedure

•For each patient visit,

•Hospital visit summary is printed for each visit

•Patient is enrolled and Patient information is made available through Patient Portal

•If the patient comes only for nurse only visit, it is documented in the progress note

•Immunization, Regular visit are also documented and updated in the portal

•Critical laboratory visits also needs to be updated by laboratory to the providers.

•Telephone encounters should also be recorded and reviewed within the same day

Monitoring

•It should be monitored at regular intervals by appropriate person

•Reports will be presented to Compliance Committee and the Board

•Anny unlocked records should be immediately communicated to appropriate persons

Existing problems

•Lack of standardized procedures/terminology

•Privacy/Security issues: this is a high concern for patients . So proper legislation/regulation should be in place

•Financial issues like huge implementation costs, maintenance costs, etc.,: This is a huge chunk of cost and revenue loss for hospital
