As we can see all the major information required is asked in the questionnaire, but it is too long and hard to get the answers as public will avoid it due to its size, a few solutions might help -
a.) Many question can be clubbed and can be asked in one like Q.9 and Q.13
b.) A questionnaire can be for specific purpose like for university sports, for university store etc. and it can be asked only from those people who are active in first survey
c.) Many questions looks irrelevant like from question 2 to question 6, they can be asked in 1 or 2 question. It is just making the survey loong and boring. It will be a burden for both public and checker
d.) Through survey it is tried to know the public response but knwhere it is mentioned to know their review or direct suggestion which might also help
