The tems decided early in the process that it did not have sufficientt data to make recommendation for process changes and decisions about system capabilities. The team developed data collection activities to get a handle on the realities of patient type within the electronic health record and then spent first four months collecting data. It collected data on the number of patients admitted to the emergency department with chest pain and transfered to the cath lab; examined the user procedure for entering and changing the admissions and transfers in the system; and finally summarizing the amountof admitting and transfering that each user performed. It did observational studies of the patient type transfer procedure, identifying how long the typical patient type transfer took from one patient type to another, the exact times of initiation and completion and the time of transfer input into the information system. It identified the number and type errors that occured concerning patient admission and transfer, and the effects those errors had on patient care and data integrity. At the end of the first four months of activity the quality council of Wester States Hospital began its annual review of the important aspects of the performance improvement program. The leader of the patient type transfer team was asked to summit a one page synopsis of the team's activities that include a brief description of team activities and a complete set of the data collected by the team.

Ans: As a member of quality council, I would like to appreciate the patient type transfer team's accomplishments so far. The team's data development activities have helped in proper collecton of data of type of patients , the time taken to enter or tranfer the patients from one type to another . This has made the classification of data based on their type ,time ,procedures in a system thus avoiding errors and helped in achieving data integrity.The team's first four months work has brought progressive results in the maintaining the record of type of patients electronically thus reducing the mistakes in identifying the type of patient in electronic health record.
