It did observational studies of the patient type transfer procedure, identifying how long the typical patient type transfer took from one patient type to another, the exact times of initiation and completion and the time of transfer input into the information system. It identified the number and type errors that occured concerning patient admission and transfer, and the effects those errors had on patient care and data integrity. At the end of the first four months of activity the quality council of Wester States Hospital began its annual review of the important aspects of the performance improvement program. The leader of the patient type transfer team was asked to summit a one page synopsis of the team's activities that include a brief description of team activities and a complete set of the data collected by the team.
