Deepwater Horizon was an ultra-deepwater, column stabilizing, dynamically positioning drilling rig off the coast of Louisiana. The drilling rig was owned and operated by Transocean and drilled for British Petroleum. On April 20, 2010, there was an explosion which led to the sinking of the rig and deaths of 11 workers, 17 others were injured. The explosion also caused the biggest oil spill in American history. At the time of the explosion, the rig was drilling an exploratory well.

a.) In February of 2009, BP had filed an exploration and environment impact plan with the Minerals Management Service, which said that it was unlikely that any accidental surface or subsurface oil spill would happen from the proposed activities. However, as is evident from the aftermath of the explosion, that was not the case. There were several signs and evidence that provided warnings to the impending accident. b.) There were a lot of safety oversights that led to the accident. The fear of reprisal caused a lot of rig workers to not reveal their concerns about equipment reliability and possible safety hazards. Workers forged entries for a confidential safety survey conducted by Transocean. The results of this survey caused the company to have a warped view of the safety conditions at the rig.
