When we see the first and last image we can see huge difference in customer's requirement or the way it is expressed. Customer wanted output present in last image but described as the first image. So eliminate this issue the project requirements should have been documented, which the sixth picture says how project was documented and as we can say its blank which means nothing. Then comes the lead understanding of it, if customer described three parts project lead understood only one (again interpretation and communication) it’s just because of interpretation of different people. Then designer again understood something and designed accordingly and explained to developer who as per his interpretation developed it. And the most important thing in IT industry or for that matter any industry even if you have created everything right but you failed to represent it and sell it well then no point of so much efforts and the better way you sell it better billing you will get which picture as well depicts in the diagram.
So it could be eliminated by using proper SRS (Software Requirement Specification) where it’s not about what person speaks, as it would be written everybody would be on same page and will not end up as Chinese whisper.
