Question

a. Patty has asthma & recently developed pneumonia that got progressively worse. Her airways eventually became obstructed and Patty had to go to the emergency room. There the doctor ordered an arterial blood gas test. Predict the results of this test. b. What you expect for the pH of Patty’s urine? c. Patty’s mother noted that Patty is hyperventilating. Explain this. d. Patty’s mother has been giving Patty large doses of vitamin C at home because she thinks that it might help Patty fight off her infection. Vitamin C is ascorbic acid. Would vitamin C help Patty?

Explanation / Answer

a) As you can see that patty has airway obstruction due to asthma and fyrther aggravted by the pneumonia. So in his blood ABG it is obvious that there will be increased PaCO2 and decreased PaO2 leading to respiratory acidosis, increased HCO3- [ as system will try to compensate the acidosis ] to compensate the respiratory acidosis.

b) As patty will develope respiratory acidosis, evident by hyperventilation and above discussed conditions, system will try to compensate this respiratory acidosis by 1) hyperventilation, 2) increasing bloof bicarbonate by means of kidney function--> Thus kidney ( by the action of type A intercaleted cell) will excrete more H+ in the urine and absorb HCO3- leading to acidic urine ( means Decreased pH of Urine ).

c) Patty has developed respiratory acidosis ( as explained above) , so system will try to compensate this acidosis by 1) Hyperventilation in order to exhale out more CO2 [ as CO2 is responsitble for the respiratory acidosis ], 2) excreting excess H+ via kidney

d) As there is already respiatory acidosis , ascorbic acid will increase the H+ concentration and thereby will increase the acidosis---> worsening the condition[ acidosis ]
