Question
You recently have started with a new client, Rebecca, a 40-year old pharmaceutical sales rep who travels frequently for business. A non-athlete for much of her childhood (she mentioned that she often struggled to keep up with the other kids), upon reaching university Rebecca took up running and now runs 4-5x per week for 5-7 km at a time. As a result, she’s managed to keep herself in reasonable shape (body composition: 21% and weight: 60kg). During your initial interview with Rebecca, you discover that she has recently returned from a 7-day business trip to Beijing (March 1st – March 7th). While there, Rebecca attempted to continue her typical running routine but found that she struggled to complete even half of her regular distance each day. Concerned by her struggles to run (which continued for several days upon her return to London), Rebecca went to see her GP who sent her off for several tests.
The results of those tests are shown below:
Max exercise test
• VO2max: 3.1 L/min
• VT:1900 ml
• Breathing rate: 44
• RER: 0.89
• HR:176 bpm
Lung function test
• FVC: 3200 ml/min
• FEV1.0: 38%
• Residual volume: 1100 ml
The GP also sends you this note,
“For the next 4 weeks, please limit Rebecca’s aerobic training to an intensity of 8METs or less”.
Based on what you know about Rebecca,
a) In order to comply with the doctor’s orders, what is the highest VO2 Rebecca would be allowed to train at?
b) At this level of intensity what would you anticipate for a RQ/RER?
c) If Rebecca trained at 8 METS for 30 minutes, how many calories should she burn?
d). At the aforementioned intensity, how many grams of fat and CHO did she burn respectively?
e) In what activities would Rebecca be able to participate?
f) What is Rebecca’s relative VO2 max
g) Looking at Rebecca’s forced vitalcapacity, you might conclude Rebecca:
h) From the values on the max exercise test, you mightconclude
i ) During her max test, what would you anticipate Rebecca’s alveolar minute ventilation to be?
j). What is the most likely cause ofher struggle to run in Beijing?
k) Based on Rebecca’s lung function test and her history, whatmightyou conclude
Explanation / Answer
a) the highhest VO2 Rebecca would be alllowed to train at is 3.1l/min.( the VO2 max is the highest or maximal oxygen uptake ). b)At highest vo2 RER will also be high because it is the ratio between the amount of carbondioxide produced and oxygen used, here the value of RER in the test is 0.89 and it indicates that mix of fat and carbohydrates are predominant fuel source, RQ it is the respiratory quotient it is calculated from the ratio of co2 eliminated and oxygen consumed it is also maximum. c)if rebecca is trained at 8mets for 30 minutes the amount of calories she burned was 240kcal (calories=METS* weight(kgs)*time(hours).) e)she can participate any activites, she can be more active in her regular running. d)as rer is 0.89 equal amount of carbohydrates and proteins are burned. fat 13.3grams and CHO is 20grams f)relative VO2 max;it is 51.6ml/kg/min, the relative vo2 max is obsolute vo2 max / weight of the person.
