You schedule him for an exercise stress test, write him a prescription for a drug, and warn him against performing any strenuous exercise until further consultation with a cardiologist. Answer the following questions with the information from the case history: 1) What is your diagnosis (specific name)? Outline the pathogenesis of this particular disorder. Answer 1:- Classical Angina Pectoris is the diagnosis.

In classical angina pectoris, the blood supply through the coronary arteries is not sufficient (The capacity of arteries usually decreased due to atherosclerotic narrowing of arteries), to augment myocardial blood flow in response to the increase in demand. When exercise or strenuous activity occurs, the myocardial blood flow demand increases, however the inability to comply this demand results in tightness or heaviness in chest, pain in chest that radiates along the shoulder and back.

Answer 2:- The drugs that are administered for treatment of angina pectoris include –

Nitrates – long acting and short acting nitrates. – For immediate dilation of arteries and hence resuming the blood flow

Beta-blockers – to reduce the increased heart rate (cardia depressant)

Calcium channel blocker – To relieve undesirable myocardial contraction inorder to decrease the induced increased demand of blood flow.

Anticholesterol drugs such as statins (atorvastatin, rosuvastatin) – To control the atherosclerosis.
