As a manager in healthcare I would see following approaches being practiced under my authority:

As per Dave’s management style, he decides that no employee will be fired for making a mistake so Milo didn’t have a choice to make this decision. Looking at some other decisions Milo made, had he be given a choice to make this decision, he might have suggested something else.

Milo was more insightful person than Dave. He looked into the “how” and “why” of things. Workplace accountability is one of the biggest and most important example from the book wherein he tries to learn more about the errors in the line of operations. He believes in knowing his employees and how they are doing at the job they do. For this he introduces performance evaluation for employees. Another management style that Milo displays is going the extra mile for the decisions he take. The company never had performance evaluation form so he went ahead and created his own.

The culture of workplace accountability is defining duty of employees to complete the responsibilities they are allotted, to accomplish the obligations mandatory for their job, and to be present for their appropriate shifts in time to justify the objectives of the company. If jobs are not finished and roles of the job are not completed appropriately, then that employee will also be accountable for dealing with the consequences. Some examples can be: Workers being present for their entire required shift; Workers finishing any jobs that have been given to them; Workers being accountable for the precise responsibilities that go along with their job; Workers being reliable in doing the right thing in all facets relating to their job; Workers moving collectively towards a mutual objectives of the company.
