Brad Martin is a 40-year-old man who comes to the pharmacy for a diabetes education class taught by the pharmacist. He would like for the pharmacist to check his blood sugar before the class begins. He was diagnosed with type 2 DM about 6 months ago. He had been attempting to control his disease with diet and exercise, but had no success. His physician started him on Orinase (tolbutamide) twice daily for about 3 months ago. He has recently experienced sweating and lightheadedness. He monitored his blood sugar and found it to be in the range of 55 to 65 mg/dL. Mr. Martin has also started taking Mylanta for GERD. Based upon the information given, assess to determine if there is a drug related problem and the possible cause. 5.Explanation / Answer
2. A1C is the glycated hemoglobin and gives the average sugar level in the body for the past few weeks or months. It is much more stable value when compared to fasting blood sugar and fasting is not required. It is a much more reliable data. Advantage over fasting blood sugar is that you need not do the test with fasting,values are not affected by stress or illness and it is more reliable.
3. Normal level of A1C is 5.7 %. Normal levels of fasting blood sugar is between 70-99 mg/dl and two hours after meals the normal levels are less than 140 mg/dl.
4. Major advantage- effective in treatment of type 2 diabetes regardless of insulin therapy and works independently. There is low risk of hypoglycemia, cause weight loss and lower systolic blood pressure.
Disadvantages may be mycotic infections, hyperkalemia, increase LDL cholesterol and are very pricey.
