The clinical pathways are a helpful tool to avoid delay in the treatment process and discharge. An initial process to be achieved as soon as possible. In the acute stage of Phase I, the patient should be treated with oxygenation, patient outcomes, clinical monitoring, assessment of patients which include observation, measurements, elimination and the patient outcome. For CHF, the patient should be assessed for the rating of pitting edema.

In phase 2, the admission of first two days, the patient should be carried out all diagnostic and laboratory investigation, medication, and treatment of appropriate interventions, and adequate nutrition. In Phase 2 the maintenance phase, the treatment and intervention of patient, mobility and activity of the patient, psychological support, and Discharge planning are included.
