Scenario Analysis Questions PCC What priority problem(s) did you identify for Mona Hernandez? PCC What potential problems could arise due to pneumonia? PCC/T&C; Discuss the importance of support from her health care team in Mona Hernandez's care. S Identify potential patient safety issues related to oxygenation and specific to Mona Hernandez's pneumonia. T&C; what other interprofessional team members should be involved in Mona Hernandez's care? Provide Mona Hernandez with education on the importance of smoking cessation and the role that smoking has on respiratory health. As part of this education, provide her with a list of smoking cessation support groups T&C;/I Concluding Questions Describe how you would apply the knowledge and skills that you acquired in Mona Hernandez's case to an actual patient care situation. The Scenario Analysis Questions are correlated to the Quality and Safety Education for Nurses (QSEN) competencies: Patient-Centered Care (PCC), Teamwork and Collaboration (T&C;), Evidence-Based Practice (EBP), Quality Improvement (QI), Safety (S), and Informatics (1). Explanation / Answer

                                                     Lung abscess

Endocarditis/pericarditis(spreading of infection to heart through blood)

                                                    Pleural effusion/empyema

                                                    Respiratory failure

Secondary bacterial lung infection post viral infection

                                                    Meningitis(spread through blood)

                                                    Septic arthritis(bacteria in joints)

Nurses – following the prescriptions without any medication error(right medicine to the right patient at right dose and time).

Checking the position of the nasal canula prongs and oxygen flow to maintain saturation above 94%

Checking for thrombophlebitis of the injection site.

Regular monitoring of Vitals (pulse, temperature, BP, respiratory rate)

Informing her about the nurses alarm so that she can call someone for help before she gets up from bed alone.

Motivating her to cough and spit the sputum and to use spirometry.

Laboratories and imaging centers: To give a reliable report as early as possible

Physiotherapist: Teach the breathing techniques to bring out maximum sputum

Specific to mona hernandes – It can cause headaches, skin irritation (already dry skin of elderly), loss of normal drive for breath

Carbon monoxide- produced replaces oxygen and carries less amount of hemoglobin to various organs which can’t be used by the body as needed.

Hydrogen cyanide – Cilia are small tiny hair like things located on the walls of respiratory system which helps to move the sputum. Hydrogen cyanide destroys the cilia and its ability to remove sputum which results in accumulation of it resulting worsening of COPD, asthma, pneumonia

Free radicals produced from smoke can harm not only the person who is smoking but the bystanders too resulting in cancer

Smoking as such can result in atherosclerosis(clogging) of pulmonary blood vessels resulting in pulmonary hypertension for which no very reliable treatment is available and lung transplantation is the last option available.

Application of knowlwdge and skills:

1.Once the patient comes proper history should be taken ,vitals should be checked followed by investigations (blood tests or imaging ) to rule out the differential diagnosis and come to a final diagnosis

2.Blood for culture and sensitivity should be collected before giving antibiotics

3.Saturation needs to be maintained only above 94% until and unless specified for specific conditions

4.to have a thorough knowledge on each disease so that disease process, progression and mangement can be explained to patient
