Botulism is an illness produced by bacterium Clostridium botulinum which produces a toxin causing weakness, blurred vision. the toxin blocks nerve function via inhibiting acetylcholine release from presynaptic membrane.

CDC 2016 study reports 205 cases  149 (73%) infant, 29 (14%) foodborne, 24 (12%) wound, and 3 (1%) of unknown etiology.
