Question

Jennifer Lange, female, 25 years old was transfused with two units of type-specific crossmatched packed RBCs during an emergency surgery. After a day leaving the hospital, she was readmitted to the hospital for complications due to a drop of hemoglobin. A new type and screen were performed. LISS was used as an enhancement media. The results of the antibody screen test are below:

What is the logical intepretation of these results?

Screen Cell Immediate Spin 37oC AHG (Polyspecific) SC I 0 0 3+ SC II 0 0 3+ Autocontrol 0 0 3+

Explanation / Answer

AHG polyspecific phase detects IgG antibodies present in the RBC. Antihuman globulins are antibodies directed against human globulins such as IgG and complement components.
AHG method it shows +3 in gel method. If no agglutinates are present, the red cells form a pellet at the bottom of the gel it mention as 0, indicating that no significant antibodies were detected. Agglutination indicates an antibody was detected. If large agglutinates are present, the red cells form a layer at the top of the gel indicated +3 or +4 etc.

So these experiment tells that RBC are react with antibody due to crossreactivity of antigen.
