Explanation / Answer

The Km of the glucose transporter is comparable in both patients. However, consistently rate of reactions are much faster in A relative to B. Though A has a low affinity for glucose as its hexokinase has a higher Km, but it makes u by its much high Vmax. Glycolysis is occurring at a smooth rate in A. the end product pyruvate also gets reduced to lactate under anaerobic conditions in A by the action of lactate dehydrogenase. However, in B the reaction rates are much slower. The body may not able to cope up with the glucose demand especially under heavy physical exercises.
