The patient types as O Rh-negative; and the screen is positive at the AHG phase. What you do? Patient records for the patient in question 6 indicate one previous pregnancy and no transfusions. The panel results show an antibody reaction at AHG with ALL cells and similar strength, but the autocontrol is negative. What do you suspect?Explanation / Answer
Although the woman was Rh-negative his womb carried a Rh positive foetaus and somehow her blood got D antigen from the foetal blood. Rh-positive recipients can receive Rh positive or Rh negative RBCs, but Rh-negative recipients should only receive Rh-negative blood. Here antibody screening test was done to detect unexpected antibody. This is done through AHG test (auto human globulin reagent). She is autocontrol negative means she has alloantibody.
