Question

an eight year old girl presents to your office with her mother. you observe the mother to be very protective and politely ask her to leave the room while you ask the young lady a few personal question in the presence of your nurse. the child reports no physical or sexual abuse but does complain of itching and burning on urination. a urinalysis is supportive for a urinary tract infection(UTI)

.what is the most appropriate question to ask the young child and the mother?

.what is the most likely cause for the UTI?

please no hand writing. thank you

Explanation / Answer

UTI is caused by the bacterium E .coli. it is common flora of the gut and external genitalia of women . So it can reach to bladder through gut or from the vagin,a and urethra during coitus. As the patient does not give any history of abuse, so the most likely cause of the infection must be poor hygiene.

The appropriate question to ask the young child is "how does she maintain her hygiene" , "does she wash her hands after bowel movements"

The mother should be advised for taking care of the hygiene of her child.
