Question

Juanita Rivera is a long-time patient of Dr. Chandin. She is well liked by the staff because she is always pleasant and flexible when it comes to scheduling. Last winter, Juanita was injured when the car she was driving was hit by a drunk driver. The drunk driver died in the crash and Juanita suffered severe head, back, and leg injuries. She was given multiple pain medications to aid in her recovery while hospitalized. During the subsequent months, Juanita worked hard with the therapists to recover most of her mobility but still suffered from residual pain. She now visits Dr. Chandin's clinic monthly for check-ups and refills on her medications. Two months ago, Juanita phoned the clinic to say she had an accident that morning when taking her pills and had dropped them in the toilet. She was apologetic in causing additional work for the clinic staff to gain a new prescription. Dr. Chandin's medical assistant visited an orthopedic clinic recently and met Juanita there. They were alone in the waiting area and so the medical assist ant asked Juanita how she was progressing. Juanita stated how she had been feeling fairly well and was able to get away for the weekend with her husband but had mistakenly forgotten her medications at the hotel at which they were staying. She would need to call the clinic for a refill. What is likely happening in this situation? What policies should exist in Dr. Chandin's clinic to deal with this problem? Should the medical assistant tell Dr. Chandin that she saw Juanita at the orthopedists' clinic? Is that a breach of confidentiality? What could be the significance of Juanita beingseen by another provider? Does Juanita fit the stereotype of a substance abuser? What would you do if you were the medical assistant?
