Evidences the hospital should gather:-
1. Number of fall injuries in the hospital campus. Because these will come under occupational illness.
2. Trends in pattern(increase/decrease) of fall cases post implementation of program.
3. Feedback from patient about the program is an good evidence of the training that has actually conducted.
Techniques and elements of performance
1.Adherence to program policy:- staff members knows the program policy and are following the protocols.
2. Records of evaluation.
3. Staff training records and mock drills of staff will indicate that training has actually taken place and staff is aware about the actions that are to be taken in such scenario.

These evidences will act as tools for improvement. Corrective actions can be taken at every step and performance can be evaluated.
Lesser number of cases of falls indicates the program is being implemented the way it has to be.
It would only be through root cause analysis that the loopholes can be determined. For e.g a case of fall is admitted, efforts to be made to find out where the incidence has happened? were the railings slippery? etc .
This approach would help in further minimizing such cases.
