1) This patient may have multiple pathological condition over the past 10 years. Currently she is suffering with indigestion, dysphagia, hematemesis, and GERD. It occurs may be in association with colon polyps as it was ruled out 10years before. Her current pathological condition could be GERD with peptic ulcer.
GERD is a chronic digestive condition in which the contents of the stomach flows back into the food pipe (esophagus). The backflow of acidic contents irritates the food pipe wall and causes GERD. It also affects the lower esophageal sphincter (LES), the ring of muscle between the esophagus and stomach.
3&4) She underwent LOWER GI SERIES -A lower gastrointestinal series is a medical procedure used to examine and diagnose problems with the colon (large intestine). X-rays are taken while barium sulfate, a radiocontrast agent, fills the colon by an enema through the rectum.
She is diagnosed to have colon polyps.
She also has cholelithiasis for which she undergone cholecystectomy.

H pylori test can be done through blood, breath, stool and endoscopy.
H pylori Antibody Test- It’s a blood test done to detect the presence of antibodies against HPylori bacteria,.A positive blood test for H. pylori antibody may indicate a current or previous infection.
A gastroscopy is a procedure where a thin, flexible tube called an endoscope is used to look inside the oesophagus, stomach and duodenum. It's also referred to as an upper gastrointestinal endoscopy.
