Question

1.State and discuss the legal and ethical considerations occurring in this scenario. Include supporting citations. 2.How can RNs support the patient’s decision when the beliefs of the patient are contrary to their own? List three (3) interventions, with supporting rationales, the RN would perform to ensure the delivery of culturally sensitive care. 3.Identify one (1) additional major religion, in which the same circumstances might also require the RN to advocate for the patient’s refusal of surgery. Describe the beliefs behind why the identified religion could pose a moral conflict for a similar patient.

Explanation / Answer

1Ans:

No physician has a right to touch the patient without his/her consent. We have to maintain autonomy of the patient. He should understand about the condition and cooperate with the doctors, at the same same time doctors should never force the patient. In this scenario patient is a Christian and he believes his Christian science practitioner would pay for him and don’t have to undergo surgery and it is not the right practice.

The ethical and legal considerations in this scenario are : 1. understand his situation and counsel well, explain him in such a way that he will convince, saying that may be the god has send them to treat him well and explain about his situation. Give spiritual and psychological support, motivate and explain about the benefits of the surgery. 2. Try to improve his voluntariness from the patient end to proceed with the surgery. 3. If he is panic and not convinced for the surgery , as a doctor saving patient life is important, if he move forward in this situation medical complications may arise.

2Ans:

A Register must support patient patient beleifs and encourage to understand the importance of medical professionals to secure their health. These are the only people who can save patient’s lives. Since he is quite young he can understand if we counsel him properly.

1.Nursing Diagnosis: Knowledge deficient Related to surgery

Intervention with rationale:

-Assess for the influence of cultural beliefs, norms, and values on the client's knowledge base. The client's knowledge base may be influenced by cultural perceptions.

-Validate the client's feelings and concerns related to previous learning experiences. Validation lets the client know the nurse has heard and understands what was said. To bring the trust on health care system

-explain about the consition

2.Nursing Diagnosis: Acute pain related to distention of the intestinal tissue by inflammation.

Intervention with Rationale:

-Pain assessment and plan for medication to reduce pain

-Check vitals periodically to know the status of the patient condition

-Administer fluids to maintain fluid balance in the body as he would be in NBM for the surgery

-Give psychological support to bring the confidence

3.Nursing Diagnosis: Fear and anxiety related to change in health status/surgery.

Intervention with rationale:

-Arrange religious counselor to obtain consent for surgery

-Give spiritual support and explain that Christian followers would get the medical treatment as when needed, that is also a facility arranged by a god to secure their health –to understand about science and faith in God , to agree for the surgery

3ANS:

Religious traditions tend to be as complex as they are long, and it is impossible to predict how any one patient or family member may understand or apply them in the context of health care.

In Muslim religion some people would think that God is only the one who can save their lives even at the time of health crisis. Some would have a belief that if we get some disease that means It’s a part of God’s plan and he is removing all sins from him so he has to bear the pain.

Muslim dietary regulation can affect patients' use of medications, especially drugs that have porcine origins or that contain gelatin or alcohol. The dietary prohibition against alcohol has occasionally raised questions about Muslims' use of alcohol-based handrubs in the hospital, but such handrubs should not ultimately prove problematic, because they do not have an intoxicating effect and are used for potentially life-saving hygiene nevertheless, a patient or family member's concern about this should be addressed thoroughly and sensitively, perhaps with the input of an imam.
