Question

W4A.1. Lab Assignment #1. Using the sample medical record, identify medical terms outlined in the 1) diagnosis and procedure section, 2) discharge summary and 3) history and physical report only. This is an exercise to determine if you can select terms from a record, and break them down into parts. All medical terms should be included, to include cells, clinician names, diseases, and disorders. You do not have to repeat terms that are listed more than once, in different areas of the record. Also, do not place any chemical terms or medications into the chart (i.e. potassium, chloride, protimes, sodium, Symmatrel, Parafon Forte, etc.) For each term, create an entry into a flashcard table (i.e., Excel Spreadsheet) to 1) identify the term 2) provide a definition, and 3) document the root, prefix and suffix. If the term has no specific pre/suffix, indicate this by placing "N/A" in the column. Flash Card Table Medical Term Definition Prefix Root Suffix Example: thalessemia a group of hereditary hemolytic diseases caused by faulty hemoglobin synthesis. n/a thalessa -emia Example: hypoglycemia Glucose deficiency in the bloodstream hypo- Glyc(o) -emia Sample Record-https://edge.apus.edu/access/content/group/362428/HIMA100%20sample%20record1.pdf
