The style of communication has to be flexible. Flexibility and hierarchical differences are the key differentiating aspects between effective and dysfunctional patterns of communication. The way we communicate with our seniors would be different from the way we communicate with our juniors. Pertaining to health-care organization communication is the most important factor which determines the patient quality outcome.

An effective pattern of communication entails flexibility; non-existence of any hierarchical differences; a proper balance between verbal and nonn-verbal communication; involvement of optimum number of stakeholders and proper place. As given in the question the interactions between a nurse and a physician at 2am when a patient requires new pain management orders means the patient is critical and the situation demands effective communication. However, the nurse may develop a feeling that hierarchical differences exist as physicians are at the top of hierarchy and this can come into play and prevent the much needed collaboration to ensure that the proper treatments are delivered appropriately. Now, the nurse keeping in mind the greater cause and her ultimate aspect of the service which is to deliver care for the patient, she would call the doctor any time of the day without any hesitation. An effective pattern of communication would encourage such communication.

A dysfunctional pattern of communication lacks flexibility; has hierarchical differences; lacks balance between verbal and nonn-verbal communication; involves too many people and the selection of place is not proper. As given in the question the interaction between a nurse and a member of dietary staff when an incorrect meal tray is just about to be given to a patient means a lapse has occured which needs to be managed appropriately. The nurse would point out the mistake to that member of dietary staff in a tacit manner and after that person is over with the servings would talk to that person in a polite manner and make him/her realize that it is a big mistake, from the next time he/she should be careful as it may cause a serious problem to the patient and they are all to serve patients to the best of their ability and knowledge. A dysfunctional pattern of communication would have led to the nurse using hierarchical differences to her advantage and may have scolded that person in front of every one which may not have guaranteed an improvement in the service. Both the examples relate strongly to the hierarchy aspect of communication.
