SURGICAL ASSISTANT ROLE PNEUMATIC TOURNIQUETS: The components of pneumatic tourniquet consists of 1)INFLATABLE CUFF:consists of rubber bladder within a plastic or fibre covering. 2)CONNECTVE TUBING:The tube which connects the cuff to the pressure device. 3)PRESSURE DEVICE:It consists of air compressor ,digital display,buttons for setting pressure, ROLE OF SURGICAL ASSISTANT:-THe surgical assistants should have proper training to demonstrate the proper using of tourniquet.He should review the polcies and regulations for using poneumatic tourniquet.the tourniquet should be tested before usng it on the patient and should follow the instructions given by the manufacterer.Non disposable cuffs and tubings should be cleaned and disinfectant before using to other person by following the manufacturer instructions.He should have knowlege of contadications before using tourniquet like, open fractures,severe crushing injuries,severe hypertension,skin grafts ,pheripheral artery disease, sickle cell anemia,DM,malignanat tumours.he should follow standard recommendations for applying tourniquet and usage of it and adjusting the pressure for adult or poaediatric inorder to prevent the the injury to the patient.Proper documentation of usage of tourniquet shoulf be recorded in patient file.

ROLE OF SURGICAL ASSISTANT IN ORTHOPEDIC SURGERY:- According to a survey most of the orthopedic toureniquets are applied to the patient in a fixed pressures irrespective of base line blood pressure of the patient.The tourniquet consists of pressure device, cuff, regulator, tubing and connectors.these alol should be checked by the physcian before applying the tourniquet.Wider cuffs provide better transmission of the tissue where as lower cuffs helps to control the artery pressure .based on patient condition mimimise the pressure potental related to the complicatons.the physical must see that the cuff should not over lie directly upon the bony prominence lke gead of the fibula in order to prevent direct compression of the nerve.He should follow the instructions befopre applying the tourniquet.The lenght of the cuff should be individulised based upon the size of the patient limb .Limbs with severe infections and poor cardiac resertve and severe traumatised limbs are contraindicated to use tourniquet.Before applicatiion of the tournequet the physcian should rue out DVT<peripheral neuropathy. he should have a proper knowledge regarding application and usage of the tourniquet.The tourniquet should be appklied carefully to the greatest circumference of the limb beace of muscle which helps in support against nerve injury.Adequate padding should be applied at the site .THe fixed inflation pressure may be 250mm of Hg for upper arm and 300mm of Hg for lower arm.Additional care should be taken in procedures involving tourniquet control on two extremities because the risk of complications and the systemic effects of tourniquet use may be increased.
