Capillary
Tendon
Vein
Artery
A 28 yr old male suffered blunt trauma to the right anterior chest. He is pale, cool, diaphoretic, and responsive to verbal stimuli. Respirations are 38 and shallow with paradoxical movement. What should you do?
Oxygenate with positive pressure ventilations
Provide high flow oxygen by non- rebreather mask
Stabilize the chest wall with bulky dressing
Call for ALS backup
A 23 yr old male received burns to his feet while “stomping out” a campfire. He is still wearing his shoes and the rubber soles and shoelaces have melted together. What should you do
Apply a moist sterile dressing
Cut his shoelaces and remove the shoes
Place his feet in a bucket of cold sterile water
Obtain a set of baseline vitals and transport
A 40 yr old male fell off a ladder and broke his leg. His left knee now bends the wrong way. You should
Check for pulse, motor and sensation in low leg
Assess skin temp of effected extremity
See if he can move his toes and ankle
Apply a traction splint
A 28 yr old man was the driver of a motorcycle that was travelling 45 MPH when it struck a parked car. He was not wearing a helmet. The patient is lying supine with a copious amount of blood around his face. E is making gurgling and breathing 4 times a in.
Ventilate via bag-valve- mask
Suction the airway
Control bleeding
Apply a c-Collar
A 47 year old male was shot in the back. He is cool and clammy and has arm weakness. Vital signs are BP 130/90, P76, R24
Tension Pneumothorax
Aortic transection
Massive hemothorax
Spinal Cord injury
Explanation / Answer
Answer 1: The wound is bleeding rapidly and spurting blood with each heartbeat. The patient most likely severed Artery. Artery damage causes blood to flow out rapidly. This has direct relation to blood pressure. Blood flow or spurting is rapid and in synchrony to heartbeat. Arterial damage can be copious and can even lead to death.
Answer 2: Provide high flow oxygen by non-rebreather mask. Since the respiration rate is dropped in this case, it is advisable to first provide with high flow oxygen to the patient by non-rebreather mask. Blunt trauma to chest often damages the rib or lung that may cause difficulty in victims to breath.
Answer 3: Cut his shoelaces and remove the shoes. Stomping out camp fire has burnt his feet while still wearing the shoes. However, the rubber soles and laces have melted. It is advisable to cut out the shoelaces and quickly remove the shoe and proceed with cold water treatment first.
Answer 4: Apply a traction splint. Commonly used support bone fractures this will help in reducing pain and also decrease the bleeding into soft tissues. It minimizes the disturbance and injury that may otherwise occur during mobility for treatment.
Answer 5: Control bleeding. The first step is to stop the profuse bleeding immediately to avoid suffering hemorrhagic shock. Profuse bleeding due to this hemorrhagic shock is life threatening.
