Assessing cognitive abilities and stage of development- Patient might have congitive impairement due to long term hospitalization and rehabilitation. Need to do some assessment of cognition., examples are-
1. Subsequent recall test.
2.Time orientaion (Asking what is the date)
3.Info (questions like "can you tell me something that happened in the news recently").
Assessing psychosocial development- It refers to patients mental and emotional health as well as one's self-concept, role development, relationships, coping stress patterns, and spiritual beliefs. below are some steps in assesing psychosocial development-
Personal Status-legal current status, self concept.
Drug History and Current Pattern of Use
Family History and Current Relationships
Mental Status Narrative-describe orientaion like Alert/Sleepy/Attentive/Unresponsive/Oriented to person/Oriented to place/Oriented to time/Confused
Strength and weakness-list out her strength and weakness.
Cultural Assessment-List any important issues that have affected the ethnic/cultural background.
Assist her in adjusting to stage of her life- Listed below are common care responsibilities-
Personal care: hepling in daily activitie
Household care: cooking, cleaning, laundry.
Healthcare: medication management, physician's appointments, physical therapy, wound treatment, injections, medical equipment and techniques.
Emotional care: companionship, meaningful activities, making positive conversation.
