The most probable structural change would have been damage to the retina. Either there could be a damage to the blood vessels or a swelling of an area called macula.

There are a lot of reasons which could explain why Roger jumped back. He may have interpreted fellow pedestrian approaching them by human echolocation. He may have been partially blind which allowed him to see it. Or it was his reflex.

There are several diseases which may result in partial or complete blindness. Stroke is one such disease where improper blood flow to the brain results in cell death.
