Lemon and milk both are never added to the hot tea together . Lemon as you know is a citrus fruit that contains Vitamin C and milk contains protein casein . So the protein in the milk gets denatured when the acidity increases due to addition of lemon juice. This results in the curdling of milk.

The curdled milk in the tea wont make you sick or ill as even the cottage cheese is made out of curdled milk, but the tea will surely not taste nice and will have a lumpy texture as the milk curdles.
