Question

Hello! My name is Ailah, and I am a first year MPH student. The class I need help with is epidemiology. The scenario is: A health maintenance organization recently formed in Madison County has 100,000 participants. An initial review of all (100,000) medical records shows that 20,000 persons had hypertension. The 1-year follow up of this population revealed 2,500 newly diagnosed cases of hypertension. Questions based on the scenario: 1. What was the initial prevalence of hypertension at the start of the study? 2. What was the incidence rate of hypertension for the one year follow up period? 3. If the number of newly diagnosed cases of hypertension continues to be about 2,500 cases per year, what would be the 5 year cumulative incidence of hypertension? 4. Estimate (from incidence and prevalence) the average duration of hypertension. I actually already have worked the problems but I just need to be sure my answers are correct. Any help you can offer is greatly appreciated!!! Hello! My name is Ailah, and I am a first year MPH student. The class I need help with is epidemiology. Explanation / Answer

1. Percent prevalence = Number of people diagnosed with disease/ Total number of people measured *100

= 20, 000/ 1,00,000 * 100 = 20%

2. During the followup period, 2,500 cases were reported. Thus, the incidence rate can be calculated as given below. Incidence rate = Number of new cases reported/ number of disease free people at the begining of study.

= 2,500/ 80, 000* 100 = 3.125%

3. If the incidence rate increases by 3.125% in the first year, 3.25% by second year (2, 500/ 77, 500* 100), 3.33% by three years, 3.44% by four years and 3.57% by five years.

Thus, the cumulative prevalnece will be equal to, 20+3.125+3.25+3.33+3.44+3.57 = 33.46%

4. The relation between prevelance (P), incidence (I) and duration of disease (D) is, P = ID

In this case, the initial prevalence = 20, 000

Incidence rate = 2, 500

Hence, Duration of disease = 20, 000/ 2, 500 = 8.

Thus, the average duration of disease = 8 years.
