Abstract on a recent warehouse accident:

The accident happened on 7th February in 2014. The name of the establishment was West Coast Distribution. A trick lift operator of the establishment was driving a Raymond lift truck through the cross aisle of the establishment's warehouse in reverse mode. The cross aisle was wet and had a puddle of water due to rains. A pallet was also stored at the cross aisle, and this along with the puddle, caused the forklift to skid. The operator's left ankle was caught between two trucks, resulting in a fracture. In its investigation, OSHA found West Coast Distribution guilty of two serious violations. For this it slapped a penalty of $8,760 on the establishment. OSHA also found the establishment guilty of five other violations and for this it slapped a penalty of $6,780 on the establishment. The OSH citations are: 01001 (penalty of $5,000), 01002 (penalty of $325), 01003 (penalty of $485), 01004 (penalty of $485), 01005 (penalty of $485), 02001 (penalty of $4,380) and 03001 (penalty of $4,380). The last two citations were related to the serious violations.

I agree with OSHA findings and citations. The West Coast Distribution was guilty of serious violations and it should have ensured that the cross aisle was clear. As a warehouse manager I would have ensured that the cross aisle is clear at all times and no pallets or other items are stored. I would also ensure that the rain water does not enter the aisle and even if it does enter, the puddle is cleared immediately.
