If the companies are within the laws of the country they are acting ethically on that region, but if they know for fact they couldn’t do what they do on homeland then they are acting unethically.

No, the companies that develop drugs shouldn’t be given immunity from lawsuits, they should play by the rules and laws of the country where they are manufacturing. The laws are there to make companies produce within the legal of each country.

No it is not ethical, companies shouldn’t only think about their profit but also they should care about helping people.

No it is not ethical because most likely the foreign country also needs the drugs and they cannot produce it on their own for different reasons

Legally they do not have an obligation, ethically it would depend on the company , I would think so, we would need to look into the mission and vision of the company to verify their ethics but in general yes companies do have an ethical obligation
