This question demands reflection on your personal behavior and the choices you want to make in your life. I will try to answer it from my perspective.
Personal code of ethics should serve as guiding principle whenever you find yourself in any kind of dilemma or in a situation when you cannot decide clearly between the options available to you. This happens many times in different aspects of life, be it personal or professional. In business setting one can encounter situations which may demand the decision to be made based on ethical considerations and not just based on the business outcomes. Some business situations may also conflict with personal interests. For example, as a Project Manager of a mega green field project, you might be in a situation when you need to decide whether you will allow bribing the officials to get the needed clearance for the project or not. In such situations, the personal code of ethics are the pillers on which you can fall back to reach a decision. The personal code of ethics should show you the light to reach a decision. As such, it is important that one must introspect and take time to formulate his/her personal code of ethics.
Code of ethics are listed in the question paper. Those are not exhaustive, there can many. You may also coin your own term and define it for yourself if that works best for you. However, It is important to understand meaning of those terms so that you can apply those codes in situations when it is demanded. I will two code of ethics I follow in my life:
1. Integrity: I define integrity as 'listing to your heart' - If my heart says something is not right, I must not do that. If my heart says I must go for it, I must strive and put efforts to get it!
2. Practicing Empathy: Empathy is about taking perspective of the one your talking with, in order to understand and connect with him/her. Here it is important that one must not judge the other while puting oneself in the shoes of other. My code of ethics is to practice empathy and to understand the perspective of the person I am talking with, not judging the person in any way. This allows me to better understand the reasons and rationalle of the others, so I can take an informed decision. This also allows me to help my subordinates achieve the best of their capabilities because if I can understand them better I can offer needed support.
Hope this helps for the assignment. However, it might be beneficial to put more efforts on this by introspecting. Also, the code of ethics may not be a fixed document that you follow all your life. As you understanding grows you may choose to ammend it and make it better every time.
