What is the probability that the forklift tire falls off given that the operator was negligent?

The total probability of forklift tire falling off = 5%

Out of this, 85% are due to operator negligence = 5%*85% = 4.25%.....................(1)

Out of rest 100-85 = 15%, 5% is the probability that operator was negligent = 5%*15%*5% = 0.0375%.......................(2)

Adding (1) & (2) is the Total probability of forklift failure due to operator negligence = 4.25% + 0.0375% = 4.2875%
