This created the ambiguity at the time of linking all the health information for the patients to maintain common EHR.
Q2) HIM director has to work with all the new acquisitions which includes Physician Office, Nursing Home, Home health agencies along with Medical center unit in order to create a centralized database to store all the health information of the project in the form of EHR.
Q3) Shared health record number will enable all facilities to access patient's complete health information with only one unique health record number. So this resolves the ambiguity problem while accessing patients records.Also this will help different unit to understand the information related to other units and it will be beneficial to provide better treatment to patients.
