For the table above, how would you normalize this to 1NF and 2NF? Also, how is this shown in table notation? In other words, what are the table notations in 1NF, 2NF, and 3NF? For example, the notation looks like this TableName (Primary Key, Field Name 1, etc).

In 1NF no two rows in table is contains repeating group of information and Table must have a primary key that is unique in each row. Generally primary key is a single column but sometimes more than one column can combine to create a primary key.
To convert given table into 1NF we have a make a primary key in this table CustomerNum +FirstName + Trip ID will become the primary key in the table and all customer must have assigned a unique customer number.
For 2NF there must not be any partial dependency of any column to primary key. For 2NF there must be single entry for each primary key value.
To convert table in to 2NF we have to spilt out table.

3NF form applies that every non-prime attribute of table must be dependent on primary key, or we can say that, there should not be the case that a non-prime attribute is determined by another non-prime attribute
