Stainless Steel (SS) thermowells (a thermowell contains a thermocouple which measures temperature) were installed in a glass-lined distillation column. The process material was a very corrosive lagrimator (tear gas). Hastelloy C was the prior MOC of the thermowells. Within one hour the thermowells failed and caused an environmental incident. Birds fell out of the sky; there were offsite complaints and cars had to be painted that were in the nearby company parking lot.
Also improper fabrication can cause failures.

When the material being installed are that much dangerous if failed they must have been thoroughly checked before installing them . Proper safety precautions must have been taken so that when the thermowell fails immediately it is prevented from spresing to the environment like shielding the place . Trained professionals must have been employed to perform the task . These precaution can prevent such incidents to the maximum extent.
